Overview
Parallel multimode at 40GbE is the purpose of the Arista 40GBASE-SR4 QSFP+ Transceiver Module, part number QSFP-40G-SR4. It has an MPO-12 connector and reaches 100 m on OM3 and 150 m on OM4.
That is the common 40GbE short-reach arrangement in data centers built around parallel fiber. Four lanes run side by side in one cable, so the cabling must be multimode MPO trunks and patch cords. Duplex LC cords do not plug into this module.
The form factor is QSFP+, the 40GbE cage style, with one port. The module will not seat in an SFP+ cage. Some switches allow a 40GbE port to be split into four 10GbE links with the right cable, but breakout is not described for this part, so check the platform documentation before relying on it.
Measure each run and confirm the fiber grade; OM3 and OM4 give different limits, and a single older segment will set the maximum for the whole path.
Arista maintains a list of supported optics for its switches. The device at the far end may have its own restrictions, so verify both.
