The Arista 100GBASE-SR4 QSFP Transceiver Module, part number QSFP-100G-SR4, is the standard short-reach 100GbE optic over multimode fiber. It reaches 70 m on parallel OM3 and 100 m on OM4, and the form factor is QSFP28.
The word parallel in the reach note tells you how it works: four lanes in each direction run side by side, which means the cabling is parallel multimode with multi-fiber connectors. Confirm the connector type with the manufacturer's data and your trunk cables, and do not assume that duplex patching will work.
The distances suit links inside a data hall. Longer runs need extended-reach multimode or single-mode optics. Measure each path and confirm the fiber grade, since the OM3 and OM4 ratings differ.
