Two duplex single-mode pairs is the layout of the Arista OSFP-400G-2FR4, according to its title: 400GBASE-2FR4 in an OSFP module with a 2 km reach.

That design differs from a module with a single duplex pair, because with two pairs per module you plan two duplex runs per port, and patching needs to provide both.
